If you are requesting a CD with images from diagnostic imaging (Radiology) or for cardiology procedures, please submit a request to Radiology and/or Cardiology by faxing your authorization to 479.968.6965. If you need additional assistance, contact the office at 479.964.9150.
Note: All authorization forms must be printed and signed manually and accompanied by a government-issued photo ID to be valid.
