How do I get copies of my medical records?
There are two ways to obtain your medical records. You may call Health Information Management at 479-968-2841 and follow the instructions, or you may visit our office in the hospital. If you visit you will need to bring your government-issued photo identification and sign a release. You will then receive your records within 48 hours, excluding weekends and holidays.
1. You may visit our office on the third floor of the hospital. If you choose to do so, you are required to bring your government-issued photo identification and sign an authorization form. You will then receive your records within 48 hours, excluding weekends and holidays.
2. Sign up for My Health Record, the online personal health record service available to you through Saint Mary’s. My Health Record allows you to store, view and update all your health information in one secure place. Only you can decide who has access to your medical history through the personal health record, which ensures your privacy.
3.You may request to receive your records by mail. To do this, download the Authorization Form below. Once completed, you may then email it, along with a copy of your government-issued photo identification, to SMMC.HIM@saintmarysregional.com titled “Medical Records Request".
